Responsibilities

PCG is seeking highly motivated and dependable Site Assessors for a minimum of 25 hours per week. Surveyors will conduct site visits to providers of home and community-based services to individuals with intellectual and developmental disabilities requiring supports to remain in the community, with the goal of evaluating barriers to community and employment integration.

Hours are variable, including regular working hours and early mornings, evenings and weekends. Ideal candidates will have a social services background with disability populations in community environments, and/or relevant social work, therapist/counseling, or nursing advanced degree or licensure (MSW, LCSW, LMFT, LMHC, RN, etc.).

Specific Responsibilities:

  • Clearly communicate purpose and scope of site visits to providers and residents.
  • Conduct site visits to provider agency service delivery settings to assess staff and residents’ interactions and residents’ living space for compliance with new federal standards for home and community-based services settings.
  • Consistency in completing survey instruments to capture observations and regularly report results and information collected back to the project team.
  • Represent PCG and the client in a professional manner.



Qualifications

Required Skills:

  • Ability to consistently interact with provider staff and residents in an appropriate, professional, and respectful manner.
  • Attention to detail with the ability to document clear observations with accurate results.
  • Creative problem-solving ability.
  • Proficiency with Microsoft Office tools and/or other online data entry
  • Dedication to accomplishing goals and overcoming challenges.
  • Flexible, self-starter possessing intellectual curiosity.
  • Excellent oral presentation and written communication skills.
  • Solid organization and time management skills.

Required Experience:

  • Associate degree required, Bachelor’s degree preferred.   
  • Direct support experience in HCBS settings desired
  • Strong social services background, preferably with home and community-based services supporting populations such as individuals with physical and intellectual disabilities.
    • Including knowledge of federal regulations regarding HCBS settings and services
  • Direct experience in and/or knowledge of California’s HCBS systems preferred
  • Prior experience conducting interviews with individuals with intellectual disabilities
  • Knowledge and experience with person-centered planning concepts is preferable
  • Survey, assessment, or compliance experience in the area of human services or a related industry.   Prior experience conducting NCI or comparable surveys

Other Requirements:

  • Site visit scheduling is variable, with 1-3-hour site visits potentially from 7:00am to 7:00pm PST, 7 days a week. Some scheduling constraints may apply to get broad sampling of visits at different times and to ensure that visits can be completed within required timeframe, at discretion of PCG.
  • Surveyors are expected to use their own vehicles for travel, with mileage reimbursement. In-state regional travel will be required to conduct site visits but will be assigned and scheduled in an attempt to cluster site visits to nearby providers. Overnight travel is not anticipated but may be necessary.
